H@N and Outreach Integration

    How many teams are Outreach and H@N together? We have just started a team that incorporates these two roles.Do people see it as two separate roles?

    What I am hoping to address is that Outreach represent Critical Care around the hospital and H@N the parent team. How do teams wear both these hats at once? How does your referral system work? Is it different at night than in the day? Is there any confusion that needed to be addressed? 

    If you are a mixed team, how many beds do you have? How many referrals a month?

    Hi Natasha.

    We are a 24/7 team here in Sunderland  and don't incorprate H&N as  routine ie  looking after tracheostomies.  We do see patients who trigger the NEWS from that speciality however the parent teams look after their own patients.

    We have a bleep system in place for anyone Triggering the NEWS for referrals and also do A&E referrals to assisst in intubations only. We see approximately 150 per month. Apart from the follow ups we see on discharge from ICCU

    The only difference in service is from 12midnight we cover the cardiac arrest bleep also until 8am

    At East Lancs we are a redesigned team incorporating traditional outreach team with out of hours. We respond to sick patients in the traditional way, but have A  more Integrated role within the out of hours cover with medical and surgical teams. We tribe urgent bleeps for junior doctors also. As an outreach nurse this integration works well and enhances the role of safety across the hospital. Message me if you would like more details.
