Electronic outreach form

    Dear All,

    I hope you all keeping well.
    I am thinking to design an electronic outreach form. Any ideas ??  This form will work similarly as the electronic incident form.

    Hi Mohammed,

    Could you clarify what you mean by an electronic outreach form? Are you looking to develop a form to use to input data onto a database, or are you going to be moving towards the use of hand held computers to collect your data.

    I'm sure many of our members will be able to give you advice and support about both.

    I am sorry for the late reply,

    I was away for sometime.

    The elctronic form has nothing to do with handheld solutions,

    Its merely to do with input data for audit and resaearch purposes.

    Any ideas welcome.

    Hi,

    I agree that the SPOT(light) study may serve your inital purposes well. SPOT(light) is collating the routine data that outreach teams already collect when assessing patinets on wards, this is a ICNARC supported prospective observational multi-centre study, however the benefits of participating are that any data inputted by your organisation to this study will be on a secure web portal available for your local purposes, enabling you to pull information for audit purposes. you will also recieve an end of study report describing your caseload, severity of illness and outcomes.
